Get started as a new patient
- Browse our provider listing to get an idea of whom you would like to choose to provide your care.
- Give us a call to confirm and discuss the availability of the provider you have chosen
- Send us over your general information (name, date of birth, address, contact information, emergency contact and insurance details)
- Please kindly read through our pre-visit checklist to ensure that you’re well prepared for your visit with us
- You will then receive your booking confirmation via the pre-check-in system. The system will send a reminder three (3) days before your visit. The mandatory paperwork will be sent over for you to fill out before your appointment
- Please send your medical records to Sellwood Medical Clinic before your visit. It is recommended to send the medical records with the filled-out mandatory paperwork.
- You will receive a booking confirmation via email when your appointment is approved
You’re all set for your first visit, and we do highly advise you to arrive 15 minutes before your appointment with us.
